Leader/Manager Offboarding Form

The following info needs to be provided by the departing employee’s Manager for any employee leaving Distinguished Vineyards or any of its wineries. Providing this information will ensure hardware, access, credit cards, etc. are closed out.

If the departing employee had a DVWP laptop issued to them, it is the manager’s responsibility to ensure it is returned within two weeks of the offboarding date.

Those employees who currently have direct deposits will receive their final check via ACH and for those who don’t, we will mail the check to their home address, therefore verification of the home address is necessary. Please note, sales incentive pays such as (tips, bonuses, commissions) that are not going to be calculated on the final day , will be processed, and sent via a check to the employee’s home address.

For Hourly Employee’s Final Paycheck (ONLY): All timecards must be approved at least 2 days prior to their last day. (For Example: if employee’s last day is Friday, you must approve the timecard by Wednesday and add the hours for Thursday and Friday to employee’s timecards if the employee is scheduled to work on those days).
