Current Employee – Employee Status / Position Change

The following info needs to be provided by the Hiring Manager for any new employee joining Distinguished Vineyards or any of its wineries.  Providing this information at least 7 days prior to start date will ensure the appropriate hardware, software and access permissions are established across all IT platforms.

If the new manager is the same as previous manager, just list name of the same manager in this field.
